- Across the top of the page, you’ll see three options: Edit Form, Publish, and Reports. Click Publish
- Review the settings displayed, and select Request Review
This will open another window, which prompts you to confirm your banking and contact information and agree to the Zone4 terms of service before you submit the form to Zone4 for approval. We review all forms usually within one business day to make sure things looks good and spot any potential problems. You will receive email notification of approval or changes required by email once your form has been reviewed.
